Blood banks

Blood bank: ducts, cold chain and the storage room

Blood bank refrigerators are built to hold 2 to 6 degrees. What is not written on their data sheet is that the effort depends on the air around them, and that air arrives through a duct.

Short answer

A dirty duct affects the storage room temperature indirectly: it reduces the flow of cold air arriving, the room warms up, the refrigerator compressors run longer and the temperature alarms trip more often, especially when doors are opened. The WHO asks for blood equipment to sit in a well ventilated room away from heat sources. The airflow, the coil and the section feeding that room are what gets checked.

Storage room: the temperature that cannot drift

The WHO manual on blood cold chain equipment sets that whole blood and red cells are stored between 2 and 6 degrees, with a temperature alarm and a continuous record. The same manual asks for refrigerators to be installed in a well ventilated room, in the coolest part of the building, away from heat and direct light, with at least 30 centimeters clear to the wall and 40 to the ceiling so air can circulate around the cabinet.

In Panama that cool part of the building does not exist without air conditioning. The room depends entirely on the duct that feeds it, and every refrigerator returns to the room the heat it pulls out of its cabinet. With four units and a plasma freezer, the room is a small heat source the air system has to overcome all day. The restricted area logic of a hospital with protocol zones applies to this room.

How a dirty duct destabilizes the room

The effect is a four step chain. A dusty coil or a loaded filter reduce the airflow reaching the room. With less cold air, room temperature rises one or two degrees, more in the afternoon. The refrigerator compressors, which must reject heat into an already warmer room, run longer cycles. And every door opening, which used to recover within minutes, takes longer and touches the alarm threshold.

Climatiza is the technical service behind this guide and works with camera inspection, an illustrated report and cleaning only when the condition justifies it, including in the case of blood banks.

An ARTI study cited by the United States Department of Energy measured that with a dust load equivalent to one year of use the coil pressure drop rose between 6 and 30 % and the equipment efficiency fell between 1 and 9 %. In a room that needs constant airflow, that margin shows on the wall thermometer before it shows on the bill.

Temperature alarms and false alerts

Alarm patternLikely cause in the air systemWhat to check
Alarms clustering between 2 pm and 5 pmAirflow too low for the afternoon heat loadCoil, filter and damper on the room branch
Alarm on a single refrigerator, the one farthest from the diffuserCold air not reaching that cornerDiffuser aim and clearance around the unit
Alarms after every door opening, with slow recoveryRoom warmer than designWall thermometer logged hourly for a week
Alarms that started after work in the corridorCrushed duct, moved damper or open access panelCamera in the section between the air handler and the room
Alarms with no pattern, with the room at normal temperatureNot the air: sensor, door or unitRefrigerator technician

The table helps avoid calling the wrong technician. If the pattern points at the air, the camera survey of the branch feeding the room shows whether airflow is lost through a leak, a crushed section or accumulated residue. If it does not point at the air, the duct is ruled out with a photo and an unnecessary cleaning is avoided.

What to check in the donor and processing areas

The storage room is the critical point, but not the only one. The donor area receives the public in street clothes with the door opening all the time. Its return carries dust and fiber which, if the system is shared, reaches the coil the cold room depends on. The processing area has centrifuges and equipment that produce heat and release fine dust when opened, as in any laboratory.

  • Confirm on the drawing whether donors, processing and storage hang from the same unit or from separate ones.
  • Donor area return: its own filter and a grille wipe every two weeks so it does not load the shared coil.
  • Platelets, which the WHO says to store between 20 and 24 degrees: check that their room or agitator does not get direct air from the diffuser.
  • Diffusers above processing benches: rim free of dust build up that could fall on bags or tubes.
  • Cold room branch damper: marked and verified after any work in the corridor.

Record for the health authority

  1. Join the room temperature log with the air log. The authority asks for the continuous refrigerator record. If the wall thermometer and the last filter change date sit next to it, the alarms are explained without argument.
  2. File the duct report with photos and drawing. Every photo carries a date, a location and whether the section was cleaned, repaired or left. Sections without access are listed with the reason.
  3. Note the clearance of each refrigerator to wall and ceiling. It is a requirement in the WHO manual and the first thing lost when the room fills with new equipment.
  4. Close each finding with a date and a person. An open finding in the duct report is a certain question at the next inspection.

The general criteria for deciding between cleaning and watching are in when a section is cleaned and when it is watched. A blood storage room resembles, in its constant airflow logic, a server room with controlled temperature, and the site planner, in the tools section, helps estimate the level of attention before calling.

Frequently asked questions

Not directly: the unit is inside the refrigerator, which has its own control. What a duct with low airflow does is warm the room, lengthen compressor cycles and bring every door opening closer to the alarm threshold. The risk appears if the refrigerator is already at its limit.

It is the most stable option, because the room stops depending on the donor return and the load of the rest of the bank. The decision is taken by management with the duct report and the alarm log in hand. If it is not possible, the room branch is balanced and flagged as priority.

Yes, the refrigerators are not touched. What gets covered is their ventilation grille and the top of the cabinet while the section above is worked on. The job is scheduled in the window with fewest door openings and the room temperature is logged before and after.

The WHO does not set a single figure for the room, it asks for the coolest, well ventilated part of the building. The exact value is set by each refrigerator’s data sheet, which states the maximum ambient temperature at which it holds its range. That figure is copied into the room log.
