Clinics and medical offices
Air conditioning maintenance in clinics and medical offices in Panama: humidity, filters and construction work
In a clinic, air conditioning has a health argument, which is why it is better to rely on a reference guideline and not on assumptions. These are the points with backing and how they apply to a medical office.
In a clinic or medical office, air conditioning maintenance rests on three recommendations from the CDC guideline for health-care facilities: control humidity, keep filters properly installed and maintained, and monitor the system following engineers’ and manufacturers’ recommendations; in addition, it recommends commissioning the system before occupying new or renovated spaces. In a coastal healthcare facility in a semitropical climate, after replacing a duct and cleaning contaminated surfaces, Aspergillus/Penicillium at a register went from 151 to under 1 fs/m³.
What the CDC guideline recommends on HVAC
| Recommendation | Category in the guideline | What it implies in a medical office |
|---|---|---|
| Engineer humidity controls into the HVAC system and monitor them to ensure proper moisture removal | IC | Review drains, pans and humidity control |
| Ensure that HVAC filters are properly installed and maintained | IB | Verify installation and condition, with no filter bypass |
| Monitor ventilation systems in accordance with engineers’ and manufacturers’ recommendations | IB, IC | Keep records of readings and checks |
| Commission the HVAC system for newly constructed or renovated spaces before occupancy and use | IC | Verify the system before opening a new or remodeled medical office |
| Ensure proper operation of the air-handling system in the affected area after erection of barriers | IB | Check the system after construction work or partitioning |
Categories IB and IC are those assigned by the guideline itself. It is a 2003 guideline written for health-care facilities; in a medical office it applies by analogy. Local health regulations should be checked with the competent authority in Panama. D-11
A measured case in a coastal healthcare facility
The authors note that the findings suggested an original source or temporary reservoir of dampness-related genera in the system, and that health-care facilities in locations with high humidity or heavy rainfall might consider high-performance air cleaning as a strategy. For a medical office, the practical lesson is different and more modest: in a humid climate, the system needs monitoring of humidity and its damp components, and an inspection shows what is there before deciding. What to document
- Filter condition and replacement log.
- Review of pans, drains and moisture sources.
- System verification before occupying a new or remodeled space.
- Camera inspection when there is persistent odor, visible mold or construction work.
- Report with photographs and prioritized recommendations.
This page is informational. It does not replace health regulations or the criteria of each facility’s infection control officer, and it makes no claims about patients’ health.

Frequently asked questions
Do a clinic’s ducts need cleaning?
Only if an inspection justifies it: substantial visible mold, vermin, obstruction or particles from the registers. In health-care spaces, any intervention should also be coordinated with the infection control officer. The criteria are in when to clean ducts and how it should be done.
How often should a medical office’s air conditioning be checked?
There is no universal frequency: the CDC guideline asks to monitor the system according to engineers’ and manufacturers’ recommendations. The specific frequency is defined in writing for each unit.
Does the filter type matter?
The CDC guideline asks that filters be properly installed and maintained. In addition, a study of 40 commercial units associated MERV 8 filters with more pathogenic bacteria and medically relevant fungi on the coil than MERV 14; it is an association, and the filter grade is reviewed together with the rest of the system.
